I have one version not listed:

Dancing In The Dark/My Hometown like version 3.
The difference is on the right, under "CBS Inc.": BRUCE SPRINGSTEEN is not written all in capital letters but it is written Bruce Springsteen

Excellent find and good observation. And thank for posting good scans. I added it to the list above as version 5. I also switched the order of versions 3 and 4.
